Important: Self-appointing surplus lines agents, viatical settlement brokers, public or all-lines adjusters, and general lines agents (2-20) or insurance agencies supervising customer representatives must log in to their own MyProfile account and click Access eAppoint Website.

eCheck, MasterCard, Discover Card, or American Express card. Visa is not currently supported by our system.
The Department will be performing maintenance in eAppoint on the weekend of July 31st and August 1st. During this maintenance window, access to eAppoint will not be available.
